Form 2 (Application for ownership, permit and plates) must be completed
by the registered owner.
The fee for replacement plates lost, destroyed, or stolen is
$5.10 per plate. However, if you need 2 plates (i.e.,
firefighter), the fee is $10.20, even if you lost only 1 plate.
Replacement plates may be issued at all Registry
of Motor Vehicles offices throughout the province.
