| Who Needs This Notice? |
|
Anyone in Nova Scotia who assists at the birth of a child,
or at a stillbirth, must submit this notice. |

Any Local Division Registrar of Births and Stillbirths properly authorized
by the Government of Nova Scotia. Most hospital Medical Records Directors
are so authorized.

| Notification Process: |
|
This notice must be completed within 24 hours of the birth
or stillbirth, and then forwarded to the medical records department of the
local hospital. The Local Division Registrar of Births and Stillbirths
in the hospital then forwards it to the Vital Statistics Section, Service
Nova Scotia and Municipal Relations. |
| Waiting Period: |

The hospital medical records department should receive it
within 24 hours. |
