Either (a) Anyone who has been
a Nova Scotia resident for at least 1 year, or (b) anyone who was born in
Nova Scotia regardless of where they now reside, who wants to acquire a
legal change of name.

An application form can be obtained
from the Vital Statistics Office of Service Nova Scotia and Municipal Relations.
When completed, it is submitted to the same office, where it is reviewed
and processed by Department staff.
Proof of identity through birth and/or marriage certificates in the
original form are required in support of the application.
If the person changing their name was either born or married elsewhere
in Canada, the notification of the name change is also sent to the Province
/Territory of their birth.
Once the processing is completed, the new name is added to the Change
of Name Register, and the applicant is sent notification by mail.

Payment at the Vital Statistics Office: can
be by cash, debit card, money order or cheque (made out to the Minister
of Finance). Visa, Mastercard or American Express are accepted.
Payment by Mail: Visa, Mastercard or American Express credit card or by cheque
or money order made out to the Minister of Finance
