An applicant can acquire an application by phone, fax, mail, by downloading from this website or by visiting the Alcohol and Gaming Division office(s).
The application is reviewed by staff to ensure that the application is completed in ints entirety. After review by Division staff, the applicant is issued the appropriate license or notified of its denial. |

This license is valid for 3 years. The renewal process is started by the Division, which will send a renewal form to the license holder before the current license expires. |
