| Who Needs This Form? |
| | Anyone who does not posses a personalized renewal notice and wishes to
renew his or her existing certificate(s).
This may be:
- Individuals who have had an address change and therefore did not
receive their personalized renewal notice.
- Individuals who have lost or misplaced their personalized renewal
notice.
- Individuals who are looking to renew a delinquent certificate.

Once your application is complete, you may submit your application along with payment;

If all requirements are met and the renewal is approved, the Certificate will be mailed to the applicant.

The Certificate is normally processed within 2 business days,
once all items that must accompany the renewal form have been received and
all requirements are met.

A Certificate is valid for the calendar year in which it
is issued (January 1 to December 31). It is renewable, and a renewal notice
will be mailed before it expires.

Annual Renewal:
Classes B, C, D, E and G: $47.75
Class F & Gas Technician License: $29.85
Renewal of multiple certificates (any class): $47.75

If renewing a delinquent certificate you must submit the
renewal fee for each missed year. Please note that if the certificate is
more than 2 years delinquent you must submit a new application form (see
Fuel Safety Technician Certificate)
as opposed to a renewal form.
