You can get an application form by contacting the office above. Along
with the completed application form, please submit the application fee,
to the office above in person, by mail or by fax.
If all requirements are met and the application is approved, the Licence
will be mailed to the applicant.

it is issued. It can be renewed. Renewal is not automatic; it is the responsibility
