Permit
Application This is used by new applicants and should be
accompanied by a letter from the applicant's employer on company letterhead
which confirms employment.
Change
of Client Information This form is used to change your registered representative status including any conflict of interest or to change your employment status excluding a change of employer.
Change
of Employer Notification This form is used by registered representatives to notify the NSLC and SNS about a change of employer.
