Department of Health - Falls Risk Assessment Framework

This document is designed to raise the awareness of falls and falls prevention and to provide the information necessary to create individualized, practical falls assessment tools and programs.

The framework has relevance in home care, acute care, and long-term care settings. It can be used to build on existing falls prevention programs or to assist in developing new falls prevention programs.
