Trauma/Critical Care PCR
This patient care record (PCR) has been adopted by the Nova Scotia
Trauma Advisory Committee to aid in the care and documentation of
critically ill trauma patients in the province of Nova Scotia.
Originally developed by the Emergency Department nursing staff of the
IWK Health Centre, it has been adapted to create a document that is
compatible with both pediatric and adult resuscitations. Numerous health care providers participated in its
design and the final document is compatible with both paediatric and adult
resuscitations. As well it can be used in any critical illness scenario, not
just trauma.
It was designed to maximise simplicity yet maintaining comprehensive indices for
resuscitation, interventions, progression of the resuscitation, documentation
and utilisation of areas for recording for all providers. It was also designed
so that, if used properly, maximum information can be extracted for registry
purposes and for retrospective research and program evaluation.
It can be
downloaded as an EXCEL document that can be formatted and changed
for the specific needs of an individual institution or can be
downloaded as a
PDF document in the form that is shown here. The PCR can be used universally by
tertiary care centres, regional institutions or smaller centres.
