Child Care Facility Annual Report

The Annual Report is a consolidated, streamlined report requesting program information and grant utilization in compliance with the Terms and Conditions of the Supported Child Care Grant (SCCG) and the Early Childhood Enhancement Grant (ECEG).

The report collects program, utilization and reporting data for the following programs:

• Early Childhood Enhancement Grant (ECEG); Utilization of funding from April 1, 2011 to March 31, 2012
• Supported Child Care Grant (SCCG); Utilization of funding from April 1, 2011 to March 31, 2012

Where an organization operates multiple sites, an Annual Report is to be completed for each full-day or part-day licensed child care facility. Please consult with your regional Early Childhood Development consultant (ECDC) in the completion this form. This report must be received by regional ECDCs no later than April 27, 2012.
