Checklist - Better Forms for Better Regulation
The form filler
- I have thought about my clients-the people who will fill in the form.
- I have considered the range of literacy levels they have.
- I have thought about how much they know about the topic of this form.
The form in general
- The form has a clear purpose.
- The title states this purpose in a way that the client will understand.
- The form gives clear instructions for completing and submitting it.
- The form includes contact information in case the client needs help.
- The form is clearly identified as a Government of Nova Scotia form.
Form particulars
Content
- Every question asks for absolutely necessary information.
- Each question asks for one thing at a time and asks for it once only.
- All necessary information is included, but no unnecessary information.
- If I have asked for a signature, I have made sure this is necessary to our process.
Organization
- I have grouped related questions together.
- I have used headings to make the sections clear, if needed.
- The order of questions will make sense to my clients.
Language
- Each question is clearly worded.
- I have used words that my clients will understand.
- I have avoided jargon and acronyms.
- I have explained any technical or legal language that I must use.
- I speak directly to my client using "you," "your company," or "your family."
- I have used the same word when I mean the same thing-for example, I have
- not used words like licence, permit, and approval interchangably.
- A fresh pair of eyes has checked my spelling and grammar.
Layout
- The form uses white space to make it inviting and not intimidating.
- The client has enough space to answer each question.
- The font is large enough for the client to read.
- If I have used graphics, they are both useful and appropriate.
Distribution
- The form is available online.
- If not a true online form, then it is at least an electronically fillable PDF (portable
- document format).
- The form is available in the right locations-online and hard copy.
